Community Resource Coordination
The Community Resource Coordination program establishes community partnerships to effectively support Well-Ahead strategic goals that support efforts to improve heart disease and diabetes prevention and management practices. The Community Resource Coordinator identifies and coordinates community-based resources and implements systems and programs that promote, maintain, and improve individual and community health. Some of the strategic goals consist of:
Improve access to and participation in ADA-recognized/AADE-accredited Diabetes Self-Management Education and Support (DSMES) programs in underserved areas.
Implement strategies to increase enrollment in CDC-recognized lifestyle change programs such as National Diabetes Prevention Program (National DPP).
Assist healthcare organizations in implementing systems to identify people with prediabetes and refer them to CDC-recognized lifestyle change programs for type 2 diabetes prevention.
Collaborate public or private sector organizations within the state to expand or strengthen availability of the National DPP and DSMES as a covered benefit among employers.
Implement systems to facilitate systematic referral of adults with hypertension and/or high blood cholesterol to community programs/resources
